If any changes are made to the information stated above, please resubmit a copy of this documents with changes marked in red to the CenterPoint Energy Service Consultant assigned to your move. Note that additional information may be requested.
Once all request information and documentation are received, please allow a minimum of 10 working business days for your proposed route to be inspected and evaluated. You will receive an agreement letter to be signed and returned along with payment in the form of a check made payable to CenterPoint Energy to an address specified by the CenterPoint Energy Service Consultant. Payment must be received by CenterPoint Energy a minumum 5 days prior to the agreed upon move date. The Contracted Moving Company shall be responsible for any additional cost not collected in the initial quote. Should CenterPoint Energy be unable to accommodate the proposed route or further evaluation is needed, you will be notified by the CenterPoint Energy Service Consultant or other company representative.
Payment of scheduled moves that are canceled within 36 hours of the start time stated above are non-refundable.

All efforts associated with this move must comply with all of CenterPoint Energy standards including all required clearances per the National Electric Safety Code (NESC) plus all Occupational Safety and Health Administration (OSHA) requirements.
